Plan members can save up to 65% off from dentistry performed by any general practitioners. Savings are even higher when it comes to preventative and diagnostic dentistry.
Who are eligible to join HealthPlus Dental Plan?
Those who are eligible to join the plan are dependents up to the age of 19, or unmarried children up to the age of 23 who are full time students.
Modes of Payment for membership
There are three modes of payment for membership available. These are the following:
1. Pay full the annual payment.
2. Automatic deductions for monthly payments from a charge card or a checking account.
3. A monthly direct billing mailed directly to your office or home with a $2 monthly service fee for this type of billing.
How to use my plan?
Upon the receipt of your application, within 7 to 10 days, you will then receive member information, a provider directory and identification cards. All you have to do is present your identification card at any of the listed providers in the directory during the time of service.
Services not covered under the HealthPlus Dental Plan Membership
Services that are not covered in the plan membership includes: hospital benefits for dental services and procedures that are performed by dental providers that are not included in the directory lists provided to members. Other coupons or discounts that are offered by any participating provider may not be used in relation to HealthPlus Dental Plan.
HealthPlus gives every new member up to 30 days to look and review the plan, if not satisfied by the plan, then all you have to do is simply cancel your membership to HealthPlus Dental Plan and you get a refund, with $7 less for administration fee.
